How it works

    Report it fast

    A form built for the moment itself: what happened, who was involved, how bad it looks. Drafts survive a dropped signal, and photos and evidence attach from the scene.

    Triage it

    Confirm the details for the incident kind — injury, near miss, dangerous occurrence, ill health and more — set the severity, and record injured persons, their injuries and lost time. Sensitive kinds stay confidential: counted in the numbers, readable only by those who should.

    Screen for RIDDOR

    A guided screen walks the reportability question — including the categories that must be reported without delay, deaths and specified injuries among them — and a negative determination is still a record. For the written-report routes, the 10- or 15-day clock is tracked: warnings before the deadline, escalation past it, and submission freezes the determination.

    Investigate and sign off

    Versioned investigations with findings, causes and evidence. Sign-off is separated by duty — the approver cannot be the person who led or submitted — and approved revisions freeze. Reopening writes revision n+1; it never edits what was approved.

    Close the loop

    Findings generate actions exactly once, each with an owner and due date set at approval. An effectiveness review lands about 90 days later — and "not effective" reopens the incident rather than filing it.

Site-scoped managers are alerted when an incident is reported; owners get one daily email covering everything they owe — and silence when there is nothing.
