How it works

Draft from a real starting point
Start blank or from the hazard library, with harmed-group presets that include care settings. Each hazard names who could be harmed and how — the way an inspector expects to read it.

Score it honestly
Likelihood times severity on your own matrix. Banding is shared across the organisation, severity floors stop a fatal hazard being scored trivial, and residual risk has to be coherent with the further controls you added.

Publish and sign
Publishing freezes an immutable version carrying the assessor’s sign-off. Later edits build the next version — the copy people acknowledged never changes underneath them.

Get it acknowledged
Send the assessment to the people it covers. Acknowledgements are version-aware, and a daily reminder chases whoever has not signed — then goes quiet.

Review on schedule
Review dates drive the register’s attention strip, and an incident involving the risk can pull the review forward to today.
